What you can refer, all in one place

Refer for a home in a village or shared home, in home support, 1 on 1 community participation, a short or medium stay, or the Rise Up group program. Not sure which fits? Ask us and we'll talk it through.

Places to live

  • Merrylands Vertical Village, a three storey apartment building with timber screens, planted terraces and a step free entry.

    A private apartment, a support team on site in the building, and Supported Independent Living (SIL) delivered to the person's plan.

    Good fit for: Someone who wants their own self contained home, with neighbours and a team who knows them well nearby.

  • The front of our shared home at Noraville, a single storey house with an accessible path to the door.

    A shared home

    A house shared with a small number of housemates, with support on site. Noraville and Hamlyn Terrace are SDA High Physical Support, Para Vista is SIL.

    Good fit for: Someone who would rather live with housemates, or who needs a high physical support setting.

Short and medium stays

  • Short term respite (STR)

    A short stay, including after hospital or when the usual arrangement needs a break.

    Good fit for: Someone who needs a safe place now.

  • Medium term accommodation (MTA)

    A place to live while housing, a plan or recovery is being sorted out.

    Good fit for: Someone approved for housing that is not ready yet.

Support services

  • Disability in home care (NDIS)

    Support hours delivered in the person's own home.

    Good fit for: An NDIS member staying where they are.

  • Aged care, Support at Home

    In home care for older people, funded through My Aged Care rather than the NDIS.

    Good fit for: An older person assessed for Support at Home.

  • 1 on 1 community participation (NDIS)

    One worker, one member, out in the community. Skills, appointments, activities and getting out and about.

    Good fit for: Someone building confidence outside the home.

  • Rise Up group program (NDIS)

    A weekly group program built on five pillars.

    Good fit for: Someone who wants routine, a group and progress they can see.

The process

What happens after you send a referral

The same four steps whichever service you are referring to.

  1. 01

    You send three things

    Who the person is, what is changing for them, and how to reach you. 

  2. 02

    We come back to you

    We acknowledge every referral within two business days, Mon to Fri, 9am to 5pm.

  3. 03

    We work out the fit

    A conversation about supports, funding and, for a home, community fit. This is where we ask for plans and reports if they exist. We give you a fit outcome as soon as we have talked it through.

  4. 04

    We start, or we say no plainly

    If we are not the right service, you get a plain answer quickly with the reason, so you can keep moving. Fit is owned by Member Success, never by occupancy.
